Management of chronic diseases such as Diabetes and Hypertension and its complications are changing and improving. In particular, we recognise that a single reading of blood pressure done in the surgery is not an accurate representation of your blood pressure.

Good blood pressure control is very important in patients with chronic diseases to protect your kidneys and heart. 

As part of the changes, we will be asking patients to take and submit blood pressure readings before they either:

• Attend the surgery for review.
• Or a review is done by the GP without needing to see the patient.
