New Forest PCN

The New Forest Primary Care Network is a collaborative group of surgeries providing a range of services to all patients within the network. The Practice joined the PCN in 2023, and it consists of the following practices:

  • Wistaria & Milford Surgeries
  • Chawton House House
  • Lyndhurst Surgery
  • New Forest Central Medical Group

As part of this collaboration we have members of the PCN Pharmacy team, Social Prescribing/Care Navigation team working from our surgery. We also have access to the PCN Paramedic Home Visiting team.
